[Comment] The evolving role of CETP inhibition: beyond HDL cholesterol

Understanding of the link between lipids and lipoproteins and cardiovascular disease has greatly evolved since the association between raised total cholesterol and heart disease was first proposed. Basic scientific findings supported by large-scale epidemiological data and randomised trials have shown unequivocally that LDL cholesterol is causally related to cardiovascular disease. Lowering LDL cholesterol (by either reduced absorption and production or increased clearance) reduces cardiovascular disease, provided that the LDL cholesterol reduction is of sufficient magnitude and duration, as shown in appropriately designed trials.
