[Comment] Transplant survival: knowing the future

Intense effort over many years has been expended on early detection of chronic injury to kidney transplants, the leading cause of allograft loss since the introduction of calcineurin inhibitor-based immunosuppression. In The Lancet, a landmark report from the GoCAR consortium describes an externally validated 13 gene marker set expressed in non-fibrotic 3 month biopsies that was independently predictive of fibrosis at 1 year.1 This classifier was superior to the current clinical and histopathological parameters used to identify patients at risk of developing interstitial fibrosis and tubular atrophy.
