We read with interest the Lancet Series on tuberculosis elimination. The role of the health-care environment in the transmission cycle of tuberculosis is underappreciated. Although increased attention to infection prevention has mitigated nosocomial tuberculosis transmission in resource-rich environments, health-care-associated transmission of multidrug-resistant and extensively drug-resistant tuberculosis is well described in low-income and middle-income countries (LMICs).1 Infection control guidelines are not universally adopted, and adherence to guidelines is not measured.
