[Correspondence] Unsuccessful trials of therapies for Alzheimer’s disease

We agree with Roy M Anderson and colleagues (Nov 25, 2017, p 2327)1 that unsuccessful trials of therapies for Alzheimer’s disease require an evaluation of how trials are designed and powered. However, we do not agree with their explanation for the trial failures so far. Their trial simulations assessed the potential for a therapy to reduce rates of transition between discrete diagnostic states: cognitively normal, mild cognitive impairment, and Alzheimer’s dementia.
